Origin of heart failure: cardiac or generalized myopathy?
نویسنده
چکیده
v the neuroendocrine response, which was once believed to be a compensatory reaction of the organism to counteract the ‘failure’ conditions, is now considered a detrimental response of the body. As such, it is recommended that this response be treated; v positive inotropic drugs, which were considered the logical treatment solution for the failing heart, do — paradoxically — increase mortality when used chronically. As such, they are now dismissed as treatment of choice; v Beta-blockers, previously considered as dangerous drugs due to their negative inotropic effects, are now thought to exert positive inotropic effects, when administered as long-term treatment; these drugs also improve prognosis. As such, their use is recommended; v the old and logical recommendation ‘of-restingin-bed’ is now considered obsolete and patients are submitted to routine physical training programmes.

Carvedilol: A Promising Drug Combined with Lipid-lowering Medications for Patients with Hypertension and Heart Failure
Background: Statins frequently cause myopathy especially in combination with fibrates, and physical activity is considered a trigger for the muscle disorder. Elevated plasma levels of creatine kinase (CK), lactate dehydrogenase (LDH) and aldolase, are the main indicators of the severity of myopathy. Carvedilol is commonly used with lipid-lowering drugs in the management of heart failure, hypert...
متن کاملEvaluation of Cardiac Mitochondrial Function by a Nuclear Imaging Technique using Technetium-99m-MIBI Uptake Kinetics
Mitochondria play an important role in energy production for the cell. The proper function of a myocardial cell largely depends on the functional capacity of the mitochondria. Therefore it is necessary to establish a novel and reliable method for a non-invasive assessment of mitochondrial function and metabolism in humans. Although originally designed for evaluating myocardial perfusion, 99mTc...
متن کاملCardiac and skeletal muscle energy metabolism in heart failure: beneficial effects of voluntary activity.
OBJECTIVE Mitochondrial function and metabolic profile of slow and fast skeletal muscles and cardiac muscle are altered in chronic heart failure (CHF), suggesting a generalized metabolic myopathy in this disease. The aim of this study was to investigate the potential beneficial effects of voluntary activity on cardiac and skeletal muscle energetics in heart failure. METHODS Heart failure was ...
متن کاملHeart attack or rhabdomyolysis?
Statins are commonly used drugs in the treatment of hyperlipidemia (HL), despite some undesirable side effects. These range from mild symptoms such as myopathy, muscle weakness and myalgia to severe muscle weakness associated with chronic myopathy and acute renal failure (ARF) as a result of rhabdomyolysis. The most serious and deadly side effect of statins is rhabdomyolysis. The case presente...
متن کاملHodgkin’s Lymphoma Presenting with Myocardial Metastasis and Heart Failure
Although involvement of the heart by malignant lymphoma is relatively common, it is unusual to be detected premortem. We report a case of Hodgkin's lymphoma who presented with systemic signs and symptoms including abdominal distension, weakness, pallor, chills and fever, hepatosplenomegaly and generalize...
